  • Send letter to participant to inform them of the receipt of appeal; letter includes details of the appeal process and three-day response time

  • Send notice to Life Director and QA Coordinator

  • Assign person(s) to resolve appeal

  • Document additional details and track history of appeal activity

  • Create letter to the participant regarding decision, process, reasons and date

  • Produce Incident/Grievance and Appeal Log for state
